Abstract

Transporting critically ill neonates by helicopter is complex and requires planning, coordination, and management. Neonates are vulnerable to stress at altitude and during rotary-wing transport. Environmental stressors can compromise clinical stability and hinder care. Successful transport begins with stabilization, particularly in cases of respiratory distress. Specialized neonatal transport systems are essential. They should be equipped with continuous physiological monitoring and mechanical support tools that maintain homeostasis throughout the flight. Aviation dynamics are also a contributor to transport-related morbidity. The Neonatal Emergency Transport Service physician's role extends beyond clinical oversight to active situational awareness of the flight environment. Collaboration with the flight crew enables the physician to monitor vertical ascent and descent in real time. This awareness can allow for requests to moderate vertical speed or alter flight paths to protect fragile neonates from stressors. Integrating this perspective into neonatal air transport protocols can significantly improve outcomes.
